How can we improve nocturnal BP assessments?

KEY MESSAGES

  • Ambulatory Blood Pressure Monitoring (ABPM) can help to assess nocturnal BP (for both nocturnal dippers and reverse dippers) by giving many advantages, like having objective measures in 24h and white-coat and masked hypertension detection.
  • ABPM is indicated to:
    • Diagnose hypertension
    • Detect white-coat and masked hypertension, nocturnal hypertension, non-dippers, and reverse dippers
    • Confirm uncontrolled and resistant hypertension, 24h BP control, symptomatic hypotension
    • Assess BP changes due to autonomic failure
  • ABPM main disadvantages are possible discomfort (particularly during sleep), and the frequent not calculation of sleeping BP using the individuals’ sleeping times.
  • Many devices have been introduced with a great potential to obtain multiple and/or continuous BP measurements for days without disturbing the patient.
